Physical form from your athlete’s physician
Copy of the athlete’s birth certificate
A recent photo
Proof of grade [A screen shot of the ParentSquare dashboard or a copy of your athlete’s report card]
Fill out the preregistration form
check/cash/money order for the signup fee - Checks made out to midlakes Junior eagles
